Please note identification verification is a legal requirement for withdrawals and that delays in providing the verification details may delay payments. Relevant forms ask for the required information ( our identity verification fact sheet provides details about the process).

Insurance forms
- Voluntary insurance application and personal health statement
(to apply for AvSuper voluntary insurance cover, including increasing income protection waiting periods) - Elect to get default insurance cover even if you are under 25 years or have a balance under $6,000

(to keep insurance cover even if your account receives no contributions for over 16 months) - Change occupation class form (or use online form)
